Discussion on identity binding / identity requirements
- EU legislation requires the following identity information (which does NOT include identity document information):

1. Data fields to be included in the vaccination certificate:
(a) name: surname(s) and forename(s), in that order;
(b) date of birth;

ICAO VDS-NC RECOMMENDS the inclusion of identity document information - but does NOT require it
HL7 / FHIR supports a multitude of identity information:
